    Located on the Ethernet card. Will contain 12 letters (between A & F) and/or numbers.


  2. Person responsible for machine.
    Name:
    Phone:
    Email:

  3. Machine or host name.
    Can be any name chosen by owner. Should have no spaces or punctuation. Dashes are ok.
    ie: Samspc or Telecom-1, Telecom-2, etc.


  4. Building and room number.
